In our office, we offer high quality care and payment options that work for all of our patients. Below you will find a list of insurance and payment options that we accept.
In-Network Insurance Plans:
- Cigna DPPO
- Connection Dental Network
- Delta Dental PPO (DPO), Premier and Federal
- Dental Health Alliance (DHA) Network
- GEHA Connection Dental Network (Federal)
- Guardian PPO
- Lincoln Financial Group PPO
- Sun Life Financial PPO
Dental Health Alliance and Connection Dental Networks are umbrella networks. Umbrella networks are not an insurance but a 3rd party administrator. Some dental plans may fall under these networks although our office is not directly contracted with them.
IF YOU ARE UNSURE WHETHER YOUR PLAN FALLS WITHIN ONE OF THESE NETWORKS, PLEASE GIVE YOUR INSURANCE COMPANY A CALL OR VISIT THEIR WEBSITE.
For OUT-OF-NETWORK insurance plans, we request that you pay us directly for the services provided at the time of the appointment and your insurance company may reimburse you for their covered amount.
Our office is not in-network with AHCCCS, HMO, or EDS plans.

Please note: Payment is due at the time of service.
As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
